As a patient or legal representative you have the right to be informed by the Childhood Cancer Registry whether and, if so, what data about you and your child have been recorded.

  • Please complete this form and sign it. We need the following information:
    • About the patient: First name, last name, social security number (AHVN13, this is the 13-digit number shown on the health insurance card), date of birth, email address and/or phone number in case we need to check anything with you, and a copy of the patient’s passport or ID card
    • About the doctor or paediatric oncologist treating (or who previously treated) the patient: Contact details: First name and last name of the doctor, name of the hospital, address, email address, phone number
  • Please send the completed, signed form and a copy of your passport or ID card to the Childhood Cancer Registry (our address is given on the form).
  • Please complete this form and sign it. We need the following information:

  • Please send the completed and signed form with a copy of the patient’s passport or ID card and a copy of your family register or the certificate showing that you are the patient’s legal representative to the Childhood Cancer Registry (our address is shown on the form).

The Childhood Cancer Registry will get in touch with you once it has received your request in writing. If data has been recorded about you or your child, the Childhood Cancer Registry will send these data to the doctor named on the contact form. Your doctor will be happy to discuss the registered data with you and explain its significance.
